What field types can I use in forms?

Eleven types: Text, Textarea, Number, Dropdown, Yes/No, Date, Signature, Info Block, Multi Select, Street Address, and Time. There is no Phone or Email field type - use Text for those. Forms do not include file upload.

Can I organise questions into sections?

Yes. Forms support named sections so you can group related questions together. For example, a boarding intake form might have sections for "Pet Health", "Feeding Instructions", and "Emergency Contacts".

What are the frequency options?

Two options: "Every appointment" sends the form each time a booking is created for a linked service. "Once per client" is per pet owner account, not per pet. After that owner completes it, it does not send again. Unlinking it from an appointment does not reset this.

Can I link a form to specific services?

Yes. Each form can be linked to one or more services, or applied to all services. A grooming waiver might only apply to grooming services, while an emergency contact form applies to everything.

Yes. Add a Signature field to any form. Clients sign directly on their device. Signed submissions are stored against the client profile and accessible from the Fields & Forms tab.

Link forms to specific services so only relevant paperwork goes out. A boarding health questionnaire goes to boarding clients, a grooming waiver goes to grooming clients. Set frequency to "every appointment" or "once per client" depending on the form.
